Have you lost your passport or identity card? Then fill out the Declaration of Missing Identity Documents.

Declaration of missing identity documents

Making a declaration of loss is required to apply for a new travel document. It is also advisable to make a declaration of loss if there is no need to apply for a new travel document because you still have another travel document. With a declaration of loss, the government registers the document number of your missing passport or Map. This registration helps in the fight against identity fraud.

Do you suspect abuse or have you become a victim of passport abuse? If so, report it first to the police and then to the municipality. Apply for a new passport as soon as possible. Read more about identity fraud on the website of the Central Reporting Point for Identity Fraud and Errors (CMI).

Have login function DigiD blocked

Have you lost your identity card? Then it is important that you have the login function for DigiD blocked. To do this, contact the DigiD helpdesk. Only Maps issued after January 4, 2021 will allow you to log in via DigiD.

Reporting abroad

Lost your passport or identity card abroad? Report it to the local police immediately. You can then apply to the Dutch embassy or consulate for an emergency document. In the Netherlands you do not have to report this to the Dutch police. With the foreign official report you can apply for a passport or identity card at the municipality.

After that, the travel document should also be registered as missing at the municipality in the Netherlands. 

Apply for emergency passport

Have you lost your passport and need to travel immediately? If so, you can apply for an emergency passport. The emergency passport is for travel only. You can apply for the emergency passport at the Royal Military Police .
