What do I need?

Bring with you when applying for an ID card:

  • All identity cards and passports you have. Even if they have expired
  • A passport photo must not be older than 6 months and must meet the passport photo requirements
  • Debit card or cash: you pay the fee at the time of application

Younger than 18 years of age

  • Yourself. You apply for your identity card in person and pick it up yourself. Your parents or guardians may come with you
  • All your identity documents and passports. Even if they have expired
  • A passport photo that meets the passport photo requirements. School photographs are not appropriate and will not be accepted.
  • ID or copy of ID from both parents or those with parental authority
  • Debit card or cash: you pay the fee at the time of application
  • Consent of both parents or those with custody

Consent of authority holder(s).

Children under the age of 12 need permission from the custodial parent(s) to apply. This is/are usually the parent(s).

If there are 2 custody holders, they must both give consent. This can be done in 3 ways.

  • Both of you come with the child. Both parents/guardians come together with the child to apply for the identity card. Both parents/guardians bring a valid ID.
  • 1 parent/legal guardian comes with a valid ID. The parent/legal guardian who does not come along completes the digital consent form.
Advice: Both of you complete the digital consent form before your visit to City Hall to apply for the identity card. For this you need DigiD.

    Give digital consent

  • 1 parent/guardian comes with you. The parent/guardian who does not come with you will fill out the consent form and bring it to City Hall. You can download the consent form below. In addition, please bring valid proof of identity from both of you.

Additional information

Map on the bus? Time for action

The municipality keeps accurate records of when your passport expires. About 5 weeks before, we will send you a card. Then it is time for action and you must apply for a new passport.

Validity

  • An adult ID card is valid for 10 years
  • An identity card for children and young people under 18 is valid for 5 years

ID Map recovered

Do you still find the old Map after applying for a new identity card? Then hand in your old Map to the municipality. This is because it is no longer valid.

Loss of more identity documents

If you have lost three or more identity documents (passport, Map and/or driver's license) within five years, it may take longer to get a new document. A rush application is not possible in that case.

Loss abroad

Have you lost your Dutch identity card abroad? Then report it to the local police as soon as possible and request a new identity card or an emergency passport (or emergency document) from the Dutch embassy or consulate. Back in the Netherlands, then request a new Map and give the official report to the municipality.

Loss of a foreign identity document

If you lose a foreign identity document, report it to the police. This also applies if you lose an identity document from Curaçao, St. Maarten, Aruba, Bonaire, St. Eustatius and Saba.

Traveling with children

Are you traveling abroad with your children? And do you have a different last name from them? Then this may cause problems at passport control at the airport. You will need proof that you are the father or mother, or that you have parental authority. More information see www.marechaussee.nl
