This form allows you to apply for the personal participation budget for pensioners.
The personal participation budget pensioners is an annual, one-time financial contribution. The contribution helps pay the costs of participating in social and community activities in your area. For example, paying membership fees to an association.
Attachments
To fully submit the application, you are required to upload several attachments. Health make sure you have these documents ready digitally so you can easily upload them during the application process.
Tip: Health that you give the files a recognizable name such as "payslip," "ID Map" or "benefit summary.
Allowed formats are: .bmp, .doc, .docx, .gif, .jpg, .jpeg, .msg, .odf, .odt, .ods, .pdf, .png, .pptx, .txt, .xls, .xlsx

If you have a partner, the partner must also sign with DigiD.
PS: If you are an authorized representative (for example, administrator or business manager) and want to submit this claim on behalf of your client, you cannot use this form. This form does not yet support the use of DigiD authorization.
