Use this form to apply for the Socially Active Bonus.
This bonus is intended for minima aged 21 years to state pension age, who have work obligations and are doing unpaid voluntary work for an extended period of time. Entitlement to this Socially Active Bonus is subject to a number of Requirements . These can be found on the website of the municipality of Leudal and are also reflected in this application form.

To fully submit the application, you are required to upload several attachments. Health make sure you have these documents ready digitally so you can easily upload them during the application process.
Tip: Health that you give the files a recognizable name such as "payslip," "ID Map" or "benefit summary.
