An individual income supplement is an allowance for people who have to live on a low income for a long time. You can receive an individual income supplement once every 12 months. There are a number of Requirements attached to this. These apply to you and any partner*.
You may be eligible for the individual income supplement if:
- you are 21 years of age or older and not yet receiving AOW
- your income in the last 24 months did not exceed the social assistance standard
- you do not have too many assets, such as savings or property
- you demonstrably have no prospect of income improvement
* You are partners if you:
- Are married or registered partners; or
- lives at the same address and:
- have a household together;
- are former spouses or former partners;
- have a child together;
- you acknowledged your partner's child or your partner acknowledged your child; or
- registered as partners elsewhere, such as with the IRS or as a co-insured with a health insurance plan.

Attachments
To fully submit the application, you are required to upload several attachments. Health make sure you have these documents ready digitally so you can easily upload them during the application process.
Tip: Health that you give the files a recognizable name such as "payslip," "ID Map" or "benefit summary.
Allowed formats are: .bmp, .doc, .docx, .gif, .jpg, .jpeg, .msg, .odf, .odt, .ods, .pdf, .png, .pptx, .txt, .xls, .xlsx
DigiD

If you have a partner, the partner must also sign with DigiD.
PS: If you are an authorized representative (for example, administrator or business manager) and want to submit this claim on behalf of your client, you cannot use this form. This form does not yet support the use of DigiD authorization.
